Release of Liability
I am aware that gymnastics and cheerleading are vigorous sporting activities involving height and rotation in a unique environment, and as such they pose a risk of injury. I understand that gymnastics, cheerleading and related activities always involve certain risks, including, but not limited to death, serious neck and spinal injuries, and injury of bones, joints and muscles. I also understand that mats, pits, and other safety equipment and apparatus provided for my protection or the protection of my child, including the active participation of a coach or teacher who will spot or assist in the performance of certain skills, may be inadequate to prevent serious injury. The risk of harm may be limited by all of the safety equipment and trained staff, but never eliminated.

I understand that participation in gymnastics, cheerleading and other related activities involves activities incidental to active participation in gymnastics and cheerleading including moving from event to event, conditioning, stretching and other activities which may leave me or my child vulnerable to reckless actions of other participants who may not have complete control over their actions or who may not see other students in the gym. My participation or the participation of my child is voluntary with the knowledge of the risks involved and I hereby agree to accept any and all inherent risks of property damage, personal injury, or death.
In consideration of my participation or the participation of my child, I hereby release and covenant not to sue R2T2 LLC, owners of USA Gymnastics World and Cheerz, the owners, staff, booster club, or other agents from any and all present and future claims resulting from ordinary negligence on the part of R2T2 LLC, or others listed for property damage, personal injury, or wrongful death arising as the result of engaging in or receiving instruction in gymnastics, cheerleading or any other activity incidental to my participation or the participation of my child. I hereby voluntarily waive any and all claims resulting from ordinary negligence, both present, past or future that may be made by me or others on the behalf of me or my child.

I further agree to indemnify and hold harmless R2T2 LLC, and the staff for any and all claims rising and as a result of engaging in or receiving instruction in gymnastics or cheerleading or any other activity incidental to my participation or my child´s participation.

I understand that this waiver is intended to be as broad and as inclusive as permitted by the laws of the State of Utah and agree that if any portion is held invalid, the remainder of the waiver will continue in full legal force and effect.
In the event of an accident involving my child, I hereby release the staff of R2T2 LLC to render any temporary first aid to my child, and if deemed necessary after evaluation by the staff, to seek medical assistance, including transportation by a staff member or ambulance to any health care facility or hospital.
I also understand that I must have, and will continue to provide proper hospitalization, health, and accident insurance coverage for my child.
I agree to allow R2T2 LLC to use photographs of my child in promotional literature which may be distributed by the gym. I also agree to allow R2T2 LLC to use photographs of my child on the facility´s website. If I choose not to have my child´s name identified in promotional literature and photographs, I will notify R2T2 LLC in writing, and that information will be retained in the student´s file.
I affirm that I am of legal age and am freely signing this agreement. I have read this form and understand that by signing this form, I am giving up legal rights and/or remedies which may be available to me for the ordinary negligence of R2T2 LLC, or their staff.

Assumption of Risk
Participation in physical activities can involve motion, rotation, and height in a unique environment and as such carries with it a certain assumption of risk. The undersigned and the participant(s) choose to voluntarily enter upon said premises under the control of said corporation, knowing their present condition and knowing that said condition may become more hazardous and dangerous during the time the participant or the undersigned is upon said premises. The undersigned and the participant(s) voluntarily assume any and all risks of loss, damage, or injury that may be sustained by the participant(s) and/or the undersigned or any property owner by them while on or upon said premises described above. The corporation may but shall not be obliged to carry insurance on the participant(s), and the existence of insurance shall not change, alter, or increase the liability of R2T2, LLC to the participant and the undersigned or affect the terms of this Release. In signing this Release, the undersigned acknowledges: a) That he/she had read thoroughly and understands completely, the terms of Registration and Release and signs it voluntarily, b) That the undersigned signing either for themselves, or as Legal Guardian is, in fact, the true and legal guardian and has the consent of the participant.

Medical Emergencies
The undersigned gives permission for R2T2, LLC, owners, officers, employees, and/or agents to seek emergency medical treatment for the participant(s) in the event the are unable to reach any parent or guardian. The undersigned also agrees that they themselves will be responsible for any financial debt incurred by said action.

Marketing Release
I understand that my child's likeness may be used in USA Gymnastics World and Cheerz ads, promotional videos, website material, or various other marketing. These images will be used for USA Gymnastics World and Cheerz purposes only, and will not be given or sold to outside companies or individuals.

Payment Policies
There is an annual registration fee due at the time of registration. Tuition is due by the first of each month and is based on flat monthly rates (unless otherwise specified in your agreement with R2T2, LLC). R2T2 LLC will send statements to all participants by email to the email account on file.

Tuition for the following month will be charged on the 20th day of the month prior. Participants who are enrolled in a class on the 20th of the month will be charged tuition for the following month. Participants who wish to not be charged for the following month´s tuition are required to either notify the front office or utilize the Drop Student Request on the parent portal on our website, www.usagymnasticsworld.com, or www.cheerzcheerleading.com. This notification is required by the 19th of the month prior to be effective for the upcoming month. Notifying a coach or simply not showing up for class are insufficient methods of making such notification.

The following forms of payment will be available: 1) Automatic Credit Card Payment, and 2) Automatic Bank Draft. Payments in full will be run on the 1st of each month. If a credit card payment transaction is declined, a second attempt will be made one week later. If a bank draft transaction is declined, we will not attempt to run it again until we have contacted the bank account owner, to avoid any extra fees. If all automatic payment attempts fail, as well as efforts to contact the billing contact for updated payment information fail, the participant will be dropped from the rolls on the 10th of the month. After that point, reconciled accounts will have an opportunity to re-enroll in class; however the desire to do so does not guarantee that the same class/schedule will be available. The re-enrollment fee of $15.00 will apply. Accounts that remain unreconciled after 30 days will be considered grounds for collections action. Declined bank draft transactions will be charged a $30.00 fee. It is the responsibility of the participant to keep credit card and bank draft payment information current and active. We honor Visa, MasterCard, Discover and American Express. All monthly tuition charges occur on the 20th of each month prior. Other miscellaneous charges, such as uniform, team fees, annual registration and equipment fees etc. occur on given fee schedule for the season. ONCE OUR CREDIT CARD AND BANK DRAFTS OCCUR NO REFUNDS WILL BE GIVEN. NO EXCEPTIONS. R2T2 LLC reserves the right to grant payment method exceptions under extenuating circumstances. Private lessons must be paid in full immediately on the date the service is performed. Payments for private lessons will be conducted through the front desk.

Credit or Bank Draft Authorization Agreement
I hereby authorize R2T2 LLC to charge my credit card or transact bank drafts for all services and products related to my family's enrollment in classes and activities at USA Gymnastics World and Cheerz, including but not limited to tuition, memberships, special events, uniforms, team fees, accessories, etc. (Note: Fees are subject to change). I understand that my credit card or bank draft will continue to be charged / transacted on a monthly basis unless I notify R2T2 LLC by either notifying the front office or utilizing the Drop Student Request on the parent portal on our website, www.usagymnasticsworld.com, or www.cheerzcheerleading.com. This notification is required by the 19th of the month prior to be effective for the upcoming month. I understand that notifying a coach or simply not showing up for class are insufficient methods of making such notification.

I have read this entire agreement and understand that I will be held fully responsible for its terms and conditions of service. I understand that once the credit card payments or bank drafts for my tuition have occurred there will be no refund given if I later decide to drop for the month billed. I agree to notify R2T2 LLC immediately of any changes in the status of my charge account including but not limited to card expiration, name change, change of address, limitation of use, loss or theft of the card, etc. In the event that the monthly bank draft is refused for whatever reason, I accept responsibility for full payment for the amount charged as well as a NSF fee of $30.00. I understand that payments for private lessons are to be conducted at the front desk. I agree to not pay any coach directly for any service.
